Because
verification is required, the following papers must be submitted
together with the registration form:
• Students:
a letter written on official letterhead and signed by a department
head or mentor and a copy of your student ID to
be presented when you pick up your registration pack at the
conference.
•
Patients:
declaration of honour that they haven’t received or unable to
receive funds from other means.
•
Delegates from low income countries:
a copy of the delegate’s passport from a low income country or a
passport from another country plus letter on headed paper from an
employer in a low income country and which
also should be presented when you pick up your registration pack
at the conference.
Only a participant with valid membership status for the coming year will be accepted as a registered conference participant. There are four types of registration:
Type 1. Full registration: Includes participation in the scientific sessions and exhibits, congress kit with the final program and proceedings, coffee breaks, lunches on Monday and Tuesday, welcome reception, and certificate of participation.
Type 2. Reduced: (reserved for patients, students and attendees from low-income countries according to OECD): Includes participation in the scientific sessions and exhibition, congress kit with final programme, coffee breaks, luncheons on Monday and Tuesday, welcome reception and certificate of attendance.
Type 3. One-day Registration: Includes participation in the scientific sessions and exhibits on that day, congress kit with the final program and proceedings, coffee break and lunches on that day, and certificate of participation.
